The Veteran Connect Project is a community initiative that focuses on Homeless Veteran Outreach in several cities in South Carolina. Inspire Real Change staff will engage Homeless Veterans and Veterans at-risk of homelessness in the community, conduct needs assessments, and develop case plans for homelessness recovery. Additionally, Inspire Real Change staff will help Veterans navigate local services, support basic needs, and coordinate housing referrals. The Veteran Connect Project is currently scheduled from 1 Jan 2023 to 31 Dec 2023. This project may extend based on community needs.
The City of Columbia Task Force to Prevent and End Homelessness partnered with Inspire Real Change in April 2022 to develop and drive solutions to reduce and end homelessness in Columbia, South Carolina. With this partnership, the Social Services Advancement Project (SSAP-001) was created to identify current programs/services, assess Social Service community integration, and initiate efficiencies/solutions to reduce homelessness.
The Community Connections Project engaged 52 unsheltered citizens in Richland County, South Carolina. Our community outreach team engage unsheltered individuals, provided health/hygiene items, and conducted on-site needs assessments. Utilizing information from the assessment, the individual's needs were prioritized to produce person-centered plans. Unsheltered citizens were directly connected with health programs/housing services to fill their needs and support homeless recovery.
The Veteran Connect Project finds its purpose in connecting Unsheltered Veterans with programs, services, and resources. The overall goal is to engage Veterans, assess their needs, and connect them with services to fill individual needs. We believe Real Change is found in filling needs, which drives the recovery process.
